The elbow strike in the second image and the larger blow-up of the same image demonstrate the possible striking surfaces for mawashi hiji zuki or round elbow strike (as noted by the little circle next to the arm in the picture.) The round elbow strike that can hit with either the flat of the forearm or the elbow. It is an obviously posed position so you can see both striking surfaces.

It is my understanding that there can be several kame or postures for the hands that have different thumb placements - not just one. So position of the thumb for the "flat hand ready post" position (hirate kame) is different from the one for striking is different for the one for sanchin. The picture is specifically demonstrating the striking aspect. So boshiken zuki kame is not necessarily identical to hirate kame.

The "fellow" in the picture is Mr. Nakamatsu. The lead editor of the text was Mr. Takamiyagi who was supported by Mr. Kanei Uechi and a panel of Uechi seniors.

I attempted to pick up the kame jars in Mr. Nakamtsu's dojo and was unsuccessful due to their weight - he, however, lifted them easily, in the usual manner, which is not the posture in the pictures.
